Trailblazer Social Care is a specialist provider of end of life home care services.
We fully appreciate the distress that our clients and their loved ones can experience when a family member is suffering from a life-threatening illness. We recognise that everyone facing a life threatening illness will require some degree of supportive care in addition to treatment for their condition.
Our end of life home supportive care services helps the client and their loved ones to cope with their condition and treatment of it. It is designed to provide support to adults and children who have acquired a life-threatening illness and who choose to remain living in the comfort and safety of their own homes. It helps our clients to make maximum use of the benefits of treatment and to live as well as possible with the effects of the disease.
Our service can also be delivered at all stages of our client’s illness, from pre-diagnosis to death and bereavement.
The overarching objective of our end of life home care service is to maintain the best possible quality of life for our clients and their loved ones. We understand that our clients do not wish to upset their loved ones and often find it difficult to express their feelings and concerns. Our experienced healthcare professionals are trained to support our clients and their loved ones in discussing and defining their feelings. They can also provide additional support, should their loved ones be required to take a break.
We ensure that all of our clients are treated as individuals, with both dignity and respect and provide them with a personalised care plans which account for their preferences, needs and choices. We also ensure that our clients receive familiar and recognised carers in the delivery of our service by selecting a limited number of carers to support our client’s needs.
Assessment of care needs and provision of on-going support
We work with the client and those significant to them to assess their care needs across the domains of physical, social, psychological, spiritual and information needs.
We recognise that supporting clients with a long-term condition or a life threatening illness can last a number of days or can continue for several years. In all events we are able to provide our clients with a wide range of services which ensure that they are comfortable and can remain as active as possible. This support includes:
Our personal care service helps to support our clients with their daily routines which may include bathing, dressing, medication prompts, and continence care and meal preparation.
Advanced Care Planning
We recognise that Death is an inevitable natural event. Planning for this event at the appropriate time is sensible and can mean that events, after death are in accordance with the deceased person’s wishes. This becomes important as the general health of the client may deteriorate to the extent that their capacity to communicate their wishes (make decisions) may have significantly deteriorated.
Trailblazer’s Social care appreciates that not everyone is willing to be involved in such planning, and we will therefore encourage BUT NOT MAKE IT A REQUIREMENT for clients to participate in discussions. This is often at commencement of care with Trailblazer Social Care. In other words participation is entirely voluntary on the part of the client and Trailblazer Social Care will never instigate the process as a result of external pressures e.g organisational needs or family.
We ensure that our client who participate in Advanced Care Planning will have been assessed as having capacity to do so. The document, called “My future Care Plan” which is essentially a statement of wishes and preferences is designed to ensure that clients feel comfortable that everything has been arranged and that their wishes will be respected.
When our client so requests, Trailblazer Social Care will give them every assistance in the preparation of the plan, with discussions handled skilfully. With the consent from the client, family members will be involved. Once formulated, the plan will be reviewed periodically and updated in line with the client’s wishes. The plan is not a legally binding document, however should be used when determining a person’s best interests in the event that they lose capacity to make decisions in the future.
Clients who are assessed as having capacity to make decisions about their future care and treatment , may also make advanced decisions about the care and treatment they would like ( or not like) to receive when they approach the final stages of their lives.
Advanced decisions must relate to a refusal of specific medical treatment and can specify circumstances. The Advanced decision will come into effect when the individual has lost capacity to give or refuse consent to treatment.
An assessment of the applicability and validity of an advanced decision is essential before it is used in clinical practice. Valid advanced Decisions, which are refusals of treatment are legally binding.
Lasting Power of Attorney
A Lasting Power of Attorney (LPA) is a statutory form of power of attorney created by Mental Capacity Act (MCA 2005). Trailblazers Social Care will support our clients who have capacity to choose a person (an attorney”) to take decisions on their behalf if they subsequently lose capacity.
Befriending and social inclusion services
We can provide our clients with an experienced Nurse, Healthcare Assistant or Support Worker to make them feel comfortable and at ease.
24 Hour Home Care Service
As the condition of our clients deteriorate, we recognise that they may need 24 hour assistance and care. In support we are accustomed to providing our clients with experienced healthcare professionals who can facilitate 24 hours per day care 7 days per week.
Our rapid response service is designed to support clients with a terminal illness during times of crisis. We appreciate that in these challenging situations when our clients are about to be discharged from hospital, they will very often rely on us to respond promptly and facilitate the immediate provision of their end of life home care. Our qualified and experienced nurses are accustomed to completing emergency client assessments and working with our clients healthcare professionals to facilitate a faster response and supporting our clients in returning home to their familiar surroundings and loved ones.
